- • SBD 1 Form (Annexure A)
- • SBD 4 Form (Annexure H)
- • RFP Declaration and Breach of Law Form (Annexure I)
- • Technical Proposal with detailed response to Technical Specification (Annexure B)
- • Pricing Schedule (Annexure D) with all cost elements
- • Proposal Form and List of Returnable Documents (Annexure E)
- • Preference Point Award Form (Annexure G)
- • Mutual Non-Disclosure Agreement signed (Annexure J)
- • B-BBEE certificate or consolidated scorecard
- • Tax Compliance Status (TCS) or CSD Report
- • Affiliations, memberships, and accreditations supporting submission
- • For joint ventures: joint venture agreement, B-BBEE certificate, TCS/CSD of each partner, proof of ownership, company registration
