- • MBD 8: Declaration of bidders past supply chain management practices (completed and signed)
- • MBD 9: Certificate of independent bid determination (completed and signed)
- • Municipal Rates and Taxes account not older than 3 months and not in arrears (or valid lease agreement / rental statement / sworn affidavit)
- • All documents must be handwritten in black ink on original forms (no typing, scanning, or pencil)
- • No electronic signatures permitted
- • Joint venture documentation must be notarized if applicable
- • 2% Corporate Social Responsibility contribution if not based in Mogale City area
