- • MBD 8 (Bidder's Past Supply Chain Practices) - original handwritten form in black ink
- • MBD 9 (Certificate of Independent Bid Determination) - original handwritten form in black ink
- • Authority of Signatory or Board resolution (if not sole proprietor)
- • B-BBEE Status Level Verification Certificate or Sworn Affidavit for EMEs/QSEs
- • Declaration Certificate for Municipal Services (completed, stamped, signed, dated)
- • Corporate Social Responsibility contribution of 2% (if not based in Mogale City jurisdiction)
- • All pages of original tender document must be included

TENDERS ARE HEREBY INVITED FOR THE APPOINTMENT OF A PANEL OF TRAINING SERVICE PROVIDERS TO CONDUCT TECHNICAL, ENGINEERING AND OPERATIONAL SKILLS (FUNCTIONAL) TRAINING PROGRAMMES AND WORKSHOPS FOR MOGALE CITY LOCAL MUNICIPALITY EMPLOYEES ON AN AS AND WHEN REQUIRED BASIS FOR A THREE (3) YEAR PERIOD.
