- • MBD 9: Certificate of independent bid determination (handwritten in black ink)
- • CSD Registration Report or Summary Report
- • Municipal Rates and Taxes account not older than 90 days and not in arrears OR valid lease agreement OR rental statement OR sworn affidavit (depending on circumstances)
- • Minimum 36 points on functionality evaluation (65 points total) to qualify for panel
- • Original documents only; no photocopies or electronic signatures
- • Handwritten completion in black ink; no pencil, erasable pen, or Tipp-Ex
- • No pages removed from original tender document
- • No joint ventures/consortia without notarized formation document
