- • Authority of signatory (for non-sole proprietors)
- • MBD 1, MBD 4, MBD 6.1, MBD 8, MBD 9 forms completed in black ink handwritten (not typed/scanned)
- • CSD Registration Report or Summary Report
- • Municipal rates and taxes account not older than 90 days and not in arrears, OR lease agreement, OR rental statement, OR municipal confirmation (for informal/rural), OR sworn affidavit
- • B-BBEE Status Level Verification Certificate or Sworn Affidavit for EMEs and QSEs (required for preference points)
- • Original documents only (no photocopies)
- • Declaration certificate for municipal services completed, stamped, signed and dated
