- • MBD 6.1: Preferential Points claim form (handwritten in black ink)
- • MBD 8: Declaration of bidders past supply chain management practices (handwritten in black ink)
- • MBD 9: Certificate of independent bid determination (handwritten in black ink)
- • CSD Registration Report or Summary Report
- • Municipal Rates and Taxes account not older than 90 days and not in arrears, OR valid lease agreement, OR rental statement, OR ward councillor confirmation (for informal settlement), OR sworn affidavit (for non-property owners)
- • Declaration certificate for municipal services (completed, stamped, signed, dated)
- • Tax Compliance Status (TCS) certificate or PIN from SARS (or CSD number if TCS unavailable)
- • B-BBEE Status Level Verification Certificate or Sworn Affidavit (for preference points eligibility)
- • 2% Corporate Social Responsibility contribution (inclusive of 15% VAT) if not based in Mogale City jurisdiction
- • All bid documents must be original, handwritten in black ink, not typed or scanned, no pencil or erasable pen, no tipp-ex
- • No pages may be removed from original tender document
- • Joint ventures must submit notarized formation document
