- • No unsatisfactory performance on previous contracts with municipality in past five years
- • Not in service of the state
- • Three Annual Financial Statements (if transaction value exceeds R10 million)
- • All mandatory forms completed and signed: Form of Bid, Form B (Declaration of Interest), Form D (Past SCM Practices), Form E (Independent Bid Determination), Form G (Municipal Accounts), Form H (if applicable)
