- • Municipal statement of account for 3 months OR signed lease agreement OR affidavit (bidder and all shareholders)
- • Audited annual financial statements for past 3 years (if bid value exceeds R10 million)
- • B-BBEE status level verification certificate or sworn affidavit (MBD 6.1)
- • Proof of address of office dealing with project (lease agreement or municipal account)
- • All pages originally signed in black ink
