- • Certified ID copies of all active directors
- • Company relevant experience (appointment letters and reference letters)
- • Company profile
- • Financial capacity documentation with original bank stamp
- • Audited financial statements for past three financial years (if bid above R10 million inclusive VAT)
- • Business fleet documentation (logbook, lease agreement, or declaration)
- • Organogram with execution team details
- • Certificate of Authority for Signatory
- • Compulsory Enterprise Questionnaire
- • Record of Addenda to Bid Documents
- • Declaration on Central Supplier Database information
- • MBD 4 (Declaration of Interest)
- • MBD 5 (Declaration for procurement above R10 million)
- • MBD 6.1 (Preference Points Claim Form)
- • MBD 8 (Declaration of Past Supply Chain Management Practices)
- • MBD 9 (Certificate of Independent Bid Determination)
