- • Latest municipal rates statement (not older than 3 months) or lease agreement or municipal confirmation of non-liability
- • Director's municipal rates statement
- • Audited financial statements for past three years (if bid value >= R10 million inclusive VAT)
- • All certified copies must not be older than six months
- • Joint Venture Disclosure Form (Annexure C) if applicable
- • Company Details (Annexure B)
- • Declaration of Interests regarding kinship/relationships with municipality employees (Annexure E)
- • Declaration of Validity of Information (Annexure F)
- • Ethics Commitment (Annexure G)
- • Past Experience documentation (Annexures A1 & A2)
- • For JV: comprehensive joint venture agreement with all parties complying and submitting relevant documents
- • MBD 5 signature required for offers amounting to or exceeding R10 million
