What you need to qualify for this tender
- • B-BBEE Status Level Verification Certificate or Sworn Affidavit (for EMEs & QSEs)
- • Proof of company registration and/or legal standing
- • Declaration of Interest form (MBD 4)
- • Company Composition form (Annexure D)
- • Audited financial statements for past three (3) financial years if offer ≥ R10 million
- • Latest municipal rates statement (not older than three months) showing no arrears exceeding three months
- • Municipal rates statement of director's properties
- • Valid lease agreement (if operating on leased property)
- • Confirmation from Municipality Finance Department (if in non-rated areas)
- • Certificate of Independent Bid Determination (MBD 9)
- • Declaration of Past Supply Chain Management Practices (MBD 8)
- • All certified copies not older than six months
- • Joint Venture Disclosure Form (Annexure C) if applicable
- • Declaration of Kinship/Relationship with Municipality employees (Annexure E)
- • Declaration of Validity of Information (Annexure F)
- • Ethics commitment (Annexure G)
